At Kiddies Hub Academy, classroom placement is based on availability. All required forms must be completed and submitted before the school can assume responsibility for your child’s care.
Please find below the list of forms that must be returned to Kiddies Hub Academy on or before your child’s first day:
- Complete Kiddies Hub Registration Form
- Copy of Birth Certificate
- Certificate of Child Health Examination
- Proof of School Dental Examination (for children 3 years and above)
